How Fluoride Works

The Caries Cycle & Fluoride's Role

Caries onset Oral acids drop pH below the critical dissolution threshold. Enamel releases calcium and phosphate — demineralization begins.
Remineralization Fluoride (F⁻) incorporates into the crystal lattice, replacing hydroxyl ions to form fluorapatite — Ca₁₀(PO₄)₆F₂.
Acid resistance Fluorapatite withstands acid to pH ~4.5 vs. hydroxyapatite's ~5.5 — a full pH unit of added protection.
Antimicrobial At low concentrations, fluoride inhibits enolase in S. mutans, reducing the bacteria's acid output directly.
Topical > systemic Varnish, rinse, and paste work locally at the tooth surface — systemic ingestion is not the active mechanism.

Clinical implication: Root surfaces dissolve at a higher (less acidic) pH than enamel. Patients with gingival recession have significantly elevated root caries risk — a fluoride varnish recommendation in this context is targeted intervention, not routine add-on.

Step-by-Step

  1. 1Explain procedure: thin coating, slightly rough texture, dissolves overnight
  2. 2Prophylaxis or scaling as needed — dry is ideal but not required
  3. 3Load brush or applicator — pea-sized amount per arch
  4. 4Apply to all tooth surfaces in thin coat; target high-risk areas (cervical, root surfaces) first
  5. 5Allow saliva contact — varnish sets on exposure to moisture
  6. 6Post-op: Eat and drink immediately ✓ · Avoid hot beverages and hard/crunchy foods for 4 hrs · No brushing for 4–6 hrs

APF vs. NaF: APF (acidulated phosphate fluoride) at pH ~3.5 can etch composites, porcelain, and sealants. Use NaF varnish (pH ~7) for patients with restorations or ceramic work.
Pineal Gland Concern

What Luke (2001) Actually Found

Jennifer Luke's 2001 study in Caries Research is real — and not debunked. It found that fluoride accumulates in the pineal gland at higher concentrations than in bone or enamel.

What it did not study: functional impairment, hormone disruption, or health outcomes. The study measured accumulation only — it drew no conclusions about harm.

"You're right that fluoride accumulates in the pineal gland — that's in the literature and I won't tell you it isn't. What the study didn't look at is whether that accumulation causes any functional change. We don't have evidence that it does."

The NTP 2020 meta-analysis found associations with cognitive effects at levels 2–10× higher than U.S. tap water. Dose is the argument — context converts concern into information.

"The IQ studies that get shared online are almost always from areas with naturally high fluoride — sometimes 5 or 10 times our level. At 0.7 mg/L, we're not seeing those signals."

  • Clinical findings that prompted the recommendation (e.g., "Miller Class I recession #22–27, early demineralization #22 cervical")
  • Evidence-based recommendation made: "Fluoride varnish recommended due to elevated root caries risk"
  • Patient's stated reason for declining — use their language in quotes
  • Information provided: studies discussed, handout given
